Notice of Medical Record Disposal

PUBLIC NOTICE

In accordance with Colorado Code Regulations, Section 1011-1 Sub Section 4.2 Chapter IV and LifePoint Policy, Colorado Plains Medical Center will be disposing medical records.

Effective March 1, 2021, Colorado Plains Medical Center will be disposing medical records of those prior to December 31, 2009, that have satisfied the required period of retention. Such records will be disposed of, with exception of records of minors which must be preserved for the period of minority plus 11 years.

If you have not been a patient at Colorado Plains Medical Center since 2009 or prior and wish to have your medical record, please request it from the HIM/Medical Records Department, prior to March 1, 2021, during the week, Monday – Friday, between the hours of 9:00 a.m. and 4:00 p.m.
