Colorado Plains Medical Center Home Care is dedicated to providing comprehensive, quality patient-centered healthcare, thus allowing patients and family members to accomplish their goals and realize their full potential. CPMC Home Care provides skilled services for the patient who requires assistance to return to independence. The physician, patient, family and staff are involved in the plan of care, while identifying attainable goals. Services are also provided on a long-term basis for disabled patients requiring skilled care who do not meet homebound criteria if payment is from Medicaid.

Department Highlights :
- CPMC Home Care will provide free blood pressure checks to any member of the public. Interested
parties can receive this service at 1006 Lake
Street between the hours of 8:00 am and
5:00 pm, Monday through Friday. - CPMC Home Care has a full-time RN who is wound
care certified and can provide specialized services
and consultations in this area.
